I’m 45, Latino, and Spanish is my first language. Lately, I’ve been overwhelmed with work and haven't had time to eat well. I have had a cough for a few weeks, which is getting worse, but have put off going to a doctor because clinics make me nervous.

This week I got a message from my health plan noting that I could get care from home… at no additional cost… with easy scheduling.

In my member portal, I easily found an appointment for that same week with a doctor who speaks Spanish.
Intake

The system knew me and my included benefits. In less than 10 minutes, I completed a simple mental health assessment and was scheduled to see the doctor I selected.
The visit experience
Day 3
Accessing my visit was easy, I just clicked a link to connect. My doctor was great! We went over my symptoms. He didn’t rush, answered my questions, and ordered some tests. Based on my initial intake form he could also see that I was overwhelmed and experiencing mild anxiety symptoms, so he gave me access to a no-cost, evidence-based Digital Behavioral Health program. I felt comfortable with next steps.
After the visit
Day 3
I received a text message to schedule my chest x-ray and blood test at a convenient location for me. While I waited for the results, I accessed my anxiety program and followed the online cognitive behavioral therapy steps to manage my stress.

Day 6
My results came back quicker than I’d thought! My doctor messaged me soon after, explaining that I had a bacterial lung infection, and he sent an antibiotic prescription to my pharmacy.
He also shared that my blood test showed I have type 2 diabetes. I was shocked, but he reassured me that it could be easily managed. He enrolled me in a digital cardiometabolic program with Dario to learn more about my diagnosis and help me manage my symptoms.
In addition to the digital program to help my general anxiety, my doctor added me to a digital behavioral health program specifically for diabetes to help me understand and process my new diagnosis. This was also covered by my health plan!
The care plan
Day 10
I have been taking my antibiotics, which have already started to clear the cough. I’m also checking in with my anxiety and cardiometabolic programs, and the tools and resources for my mental and physical health needs make me feel fully supported and better able to manage my conditions.
Follow up
1 month later
At my next appointment, my doctor updated me on the progress I was making. My cough was gone! He could also access my digital mental health programs for diabetes and anxiety and see that I was feeling much better. He encouraged me to keep up the good work and continue with my program recommendations.

My doctor sent me a message to check in and schedule a follow-up, and ordered another blood test for me.
In my visit, he reviewed my lab results and digital program feedback.
He said my anxiety was improving as well as my A1C levels. That my tracked increase in physical activity likely helped. He also said I was tracking eating more carbs than recommended for someone with diabetes, and told me to focus on foods lower in sugar and carbs. I took note—my app alerted me too!
Annual visit
1 year later
I received a text reminder to schedule my annual wellness visit. Ahead of my appointment, my doctor ordered tests to see my progress.
We went over the results in the visit. Turns out, I reduced my A1C levels by 1.4%! I’m almost at a healthy range because I focused on diet and exercise, and will continue the cardiometabolic program.
My anxiety is well managed, and I’m using Digital Behavioral Health when I feel stressed.
